Before your visit

ADMISSION FORMS

Admission Forms must be completed one week before your first visit.

Information that you will need to complete the admission process includes: 

  • Your health fund details, Medicare number and concession or health card information.

  • The contact details of your GP and your regular pharmacy.

  • A list of your current medications and medical conditions and whether you have any allergies.

Part of the admission process will also include a questionnaire about your home environment to help us plan for the visit and ensure it goes as smoothly as possible.   

WELCOME CALL

Your doctor will send a referral to our team with some of your details. A Remedium staff member will call you to welcome you to our service, confirm your details, book your first visit and go through some information. We call this your Welcome Call.  

If you are ready, you can complete your admission form with our team member during your Welcome Call. Otherwise, the admission form is online or you can book in an admission call at a convenient time.

GIVING YOUR CONSENT

You will be required to sign a consent form either at your first visit or via the admission form online. The admissions team or your treating nurse will go through the form with you if you require. Please make sure you understand the form and ask any questions you may have.  

SCHEDULING YOUR VISIT

During your Welcome Call, our team member will work with you to find the best day and time for your visit. We hope to accommodate the scheduling needs of all our patients however it may not always be possible. If you have recurring treatment visits, then the day and/or time will remain the same.  

CONFIRMING YOUR VISIT

Two days before your visits you will receive a text message or phone call from the Remedium team checking if you have had your blood tests (if required) and confirming your visit.  You will need to respond to this message for your visit to go ahead.

During your visit

During your visit we ask that you provide a clean surface for the nurses to set up their equipment. Ideally this would be a dining table or bench top.  

You can be involved in your care by:

  • Making sure we have washed our hands.  

  • Reminding the nurses to confirm your identity. 

  • Asking questions about your treatment especially if something looks different that what you normally have.  

WHEN THE NURSE ARRIVES THEY WILL

  1. Identify themselves at the door. 

  2. Ask you to tell them your name and date of birth before entering your house 

  3. If its the first time they are entering your house they may ask you to show them the nearest sink and where you would like to be treated. 

  4. Wash their hands with soap/alcohol based sanitiser many times during their visit.  

  5. Place their bags and equipment in a clear space either on the floor or on the bench/table top you have cleared for them (they might need to make a few trips to the car).

  6. Set up and unpack their equipment including the medication and infusion pump. 

  7. Ask you some questions about yourself, how you are feeling and your medications. 

  8. Ask you for your name and date of birth.  

  9. Give you your treatment. 

  10. Take notes and record your vital signs such as your blood pressure, temperature, heart rate.  

You have the right to:  

  • Be treated with dignity and respect.   

  • Receive professional and confidential care that respects your culture, beliefs, gender, age and needs.   

  • Refuse treatment and/or withdraw from services provided at any time.   

  • Involve an advocate of your choice (someone to support and/or speak for you) during your care. 

The Remedium Healthcare team will:

  • Organise mutually convenient times for your visit where possible.

  • Advise you on how long the visit is likely to take. 

  • Apply appropriate, quality services.

After your visit

FEEDBACK

Remedium Healthcare aims to provide you with high-quality, patient-centred care. During or at the end of your treatment course your nurse will provide you with a Feedback form to complete and return.

FEES AND CHARGES

Remedium Healthcare is a private health service and there may be charges associated with your visit or your medications. The cost to you will be explained during your Welcome Visit.
